Our first offering deploys virtual mental health services using Collaborative Care, an evidence-based behavioral health care model. We will scale this virtual care model using technology, and build the foundations to drive outcomes focused on value-based care.<br><br><br><strong>About the role
</strong></p> <p>Our Behavioral Health Care Managers are the consistent, trusted advocates at the very center of our care model. By providing targeted behavioral health support that integrates with the patient’s physical healthcare, they serve as the vital bridge that unifies the entire care team—from primary care provider to psychiatric consultant—around a single, compassionate plan. <br><br>This is a fully remote opportunity supporting one or more of our partner practices, working Monday-Friday.<br><br><br><strong data-start="145" data-end="161">Please note:</strong> This role is part of our evergreen hiring strategy, where we proactively identify and engage talent for ongoing and anticipated needs across our team. We appreciate your patience and look forward to connecting.<br><br></p> <p><strong>Core Responsibilities</strong></p> <ul> <li>Support the mental health of patients with cancer on an assigned patient caseload</li> <li>Provide brief behavioral interventions tailored for the cancer setting</li> <li>Screen and assess patients for common mental health and substance abuse disorders</li> <li>Facilitate patient engagement and follow-up care</li> <li>Provide patient education about common mental health and substance abuse disorders and the available treatment options </li> <li>Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications </li> <li>Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment</li> <li>Develop a trusted, effective working relationship with the partner oncology practice team</li> <li>Work within partner oncology groups to support patients, clinicians, and practice staff with the identification and intake of patients needing behavioral health support</li> </ul> <p> </p> <p><strong>Qualifications</strong></p> <ul> <li>You hold one of the following clinical licenses: <ul> <li>Licensed Clinical Social Worker (LCSW)</li> <li>Licensed Professional Counselor (LPC)</li> <li>Licensed Professional Clinical Counselor (LPCC)</li> <li>Licensed Marriage and Family Therapist (LMFT) </li> </ul> </li> <li>Dedicated to providing evidenced-based care and have a demonstrated ability to collaborate and communicate effectively in a team setting</li> <li>You enjoy working in a dynamic, fast-growing environment where priorities evolve, processes are continuously improved, and adaptability is essential</li> <li>Prior experience interacting with physicians and collaborating with a multidisciplinary team</li> <li>Have a passion for delivering quality behavioral health care</li> <li>Ability to operate with a high degree of autonomy</li> </ul> <p><strong><br>Compensation and Benefits</strong></p> <ul> <li>Competitive Compensation </li> <li>Medical, dental, vision benefits</li> <li>Flexible PTO</li> <li>401(k)</li> <li>Access to and mentorship from leading clinicians and experts in psycho-oncology and integrated behavioral health</li> </ul> <p> </p>
